An exciting opportunity has arisen for an enthusiastic and experienced AHP to join the Homefirst team in North. This position is open to Occupational Therapists who have experience of working in the community and wish to develop thier assessment, care planning and reablement skills whilst being part of a dynamic, progressive, integrated multiagency, multidisciplinary team.
The Homefirst team is part of the integrated community team in the North comprising of registered and non-registered nursing, therapy and administrative staff working collaboratively with partner organisations to support the intermediate care needs of our local population.
You will be responsible for assessment, planning and implementation of care and assist in the interpretation of information relevant to the patient’s condition/progress, identify problems and needs of patient and set realistic goals.
You will need to have excellent organisational skills combined with a wide knowledge base and experience of working in older person’s services.
We will offer a supportive working environment in which to develop your clinical and interpersonal skills, a structured in-service training programme; appraisal and supervision system.
CPD is actively supported through regular supervision and staff training
The team are based at Carew House, Bodmin but we cover the geographical area of north Cornwall, from the Devon border down the north coast to Wadebridge through Bodmin and Lostwithiel to Fowey, Par and St Blazey on the south coast so a UK driving license and access to a vehicle for work use is essential.
This is a part time post, days are planned over a 7 day rota, so this post does involve regular weekend working and the successful candidate must be flexible with their working days. We cover the hours of 8-8.
